03/31/2027. Continued employment is contingent upon adequate
funding and the need for the position as determined by the agency.**
Job Duties:
This HSS VII position reports to the Hightower unit while providing coverage for other units, as needed. You will provide direct case management to inmates with human immunodeficiency virus (HIV) or acquired immune deficiency syndrome (AIDS) who are preparing for release from prison. You will work with identified inmates pre and post release and up to 180 days.
The HSS VII is tasked with identifying barriers to a successful transition to the community and putting together a continuity of care plan using motivational interviewing and research of medical records and community resources Once a plan is made, post-release strategies begin. Relationships with our community partners are imperative. Post-release appointments are made prior to release with follow ups to the clinic and the client to ensure attendance. The HSS VII will continue to follow-up with clients 30, 60 and 90 days post release.
You will also keep records up to date, complete reports, correspondence, and review data to prepare summaries. This role requires you to stay organized, make sound decisions, and work independently while still collaborating with others when needed.

You have knowledge of HIV barriers to services.

You are proficient in Microsoft Office 365 applications and can use a computer effectively for documentation, data management and communication.
You can build productive relationships with unit staff, unit medical, providers, community partners and community resources.

You have knowledge of community resources.
You are self-motivated, take initiative and can remain on task independently.
You have 4 years of related work experience and a bachelor's degree.
You have criminal justice experience.
You have experience working with (volunteer work counts) people with HIV.
