THE POSITION
As a Community Health Nurse with the Department of Health (DOH), you will help strengthen and support healthier communities throughout the Southwest District. This role gives you the chance to apply your expertise in public health and education while partnering with dedicated colleagues and engaged community organizations. Your work will lift up vital programs that protect health, expand access, and respond to emerging needs across the region. DESCRIPTION OF WORK
This role focuses on assessing community needs, planning and delivering public health services, and supporting local health initiatives. Your work helps strengthen public health programs across the Southwest District. As a Community Health Nurse, you will perform the following duties:Community Assessment: Analyze county level health data to identify needs and resourcesProgram Coordination: Link program resources with community partners to support local health activitiesInitiative Development: Develop public health initiatives using protocols, guidelines, and community dataDisease Monitoring: Monitor communicable and vaccine preventable diseases using public health information systemsHealth Education: Provide education programs that address a variety of public health topicsClinical Services: Deliver clinical services for immunizations and specimen collection to promote patient health and prevent or control diseaseInterested in learning more? Additional details regarding this position can be found in the position description.Work Schedule and Additional Information:Full-time employment, 37.5 hours per weekWork hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.Irregular Schedule: Work infrequently requires statewide travel and irregular/flexible work hours including overnights, weekends, and holidays in accordance with policy.Telework: You will not have the option to telework in this position.Salary: In some cases, the starting salary may be non-negotiable. You will receive further communication regarding this position via email.
