THE POSITION
Are you interested in protecting the elderly in your community? Lehigh County Aging and Adult Services is searching for an Aging Care Manager 3 to join the Protective Services Unit. This is an essential team that provides specialized casework to at-risk adults aged 60 and older under the Older Adults Protective Services Act. DESCRIPTION OF WORK
As an Aging Care Manager 3, you will be providing protective services to older adults who are at risk of abuse, neglect, exploitation, or abandonment and who may be struggling to maintain a safe and healthy independent lifestyle. Work involves investigating referrals; gathering psychological, medical, and financial information to determine service eligibility; preparing case histories, reports, correspondence, and documentation; and maintaining complete and accurate case records within the Pennsylvania Department of Aging’s WellSky system. You will have the opportunity to work closely with consumers, families, social workers, physicians, courts, attorneys, and law enforcement to obtain assessments as well as provide casework and advisory services that support problem resolution and consumer well-being. You will also be collaborating with professional staff through joint home visits, case conferences, and team meetings, and may include working with in-house RN consultants for nursing assessments. Additional responsibilities include:Arranging for representative payee services when appropriate Recommending cases requiring court intervention, including guardianships, protective services actions, and mental health commitmentsPreparing documentation, providing court testimony, coordinating with attorneys and financial institutions, locating relatives, and completing required court reportsServing as a liaison with hospitals, clinics, social agencies, courts, law enforcement, and community resources to ensure coordinated service deliveryWork Schedule and Additional Information:Full-time employmentWork hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.Telework: You will not have the opportunity to telework.Salary: In some cases,
