About the Position

This is a time limited position. Special Projects provides leadership, coordination, and operational support for strategic initiatives within the Office of Adult Services, with primary responsibility for supporting the Heart Disease and Stroke Prevention Program and WISEWOMAN while leading cross-program initiatives that strengthen collaboration across multiple CDC-funded chronic disease programs.
Major responsibilities include but are not limited to: -Provide leadership and management of special projects that support the Heart Disease and Stroke Prevention Program, WISEWOMAN, Diabetes Prevention and Control Program, and other Office of Adult Services initiatives. Lead the development, implementation, and continuous improvement of the Office of Adult Services' cross-program referral system to strengthen coordination among Heart Disease and Stroke Prevention, WISEWOMAN, Diabetes Prevention and Control, Tobacco Control, Breast and Cervical Cancer Program, Comprehensive Cancer Control Program, and other Office initiatives. Develop standardized referral policies, workflows, screening tools, and tracking processes that improve clinical-community linkages, increase access to preventive health services, and promote integrated, person-centered care. Coordinate office-wide strategic initiatives that improve operational efficiency, communication, and collaboration across multiple chronic disease programs. Provide leadership for social drivers of health initiatives by coordinating community resource referrals, developing partnerships, and supporting strategies that improve access to care and reduce barriers to health. Assist the Director of Heart Disease and Stroke/Compliance Manager with implementation of CDC cooperative agreement activities, strategic planning, program monitoring, performance improvement, and special projects. Coordinate and monitor contracts, procurement activities, invoices, budgets, and other administrative functions to ensure timely completion of program deliverables and compliance with federal and state requirements. Collaborate with Epidemiology, Evaluation, Fiscal, Procurement, Information Technology, and other Office of Adult Services staff to improve coordination of program activities and strengthen accountability. Lead technical assistance efforts for healthcare systems, community-based organizations, community health workers, and other partners to improve implementation of evidence-based chronic disease prevention strategies.
