THE POSITION
Located in Honesdale, PA, Wayne County Area Agency on Aging is seeking a compassionate, organized individual to help older adults access services and supports that allow them to remain active, safe, and independent in the community of their choice. DESCRIPTION OF WORK
This position focuses on assessing needs, coordinating services, and supporting older adults and eligible individuals through clear guidance and responsible care management. Work involves maintaining cases, completing assessments, and assisting consumers with information about available programs. As an Aging Care Manager 2, you will perform the following duties:Care Assessment: Complete intake and reassessment duties and document information in the appropriate systemsService Coordination: Arrange needed services and work with community partners to support consumer needsProgram Guidance: Explain program eligibility, limits, and procedures to consumers and familiesCase Maintenance: Manage caseload tasks such as reports, records, and required follow-up contactInformation Review: Interpret requests from professionals and determine appropriate referralsWork Schedule and Additional Information:Full-time employmentWork hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.On-call hours will be requiredOvernight or out of county travel for meetings or training will be requiredTelework: You will not have the option to telework in this position.Salary: In some cases, the starting salary may be non-negotiable.You will receive further communication regarding this position via email. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONSMinimum Experience and Training Requirements:Six months as an Aging Care Manager 1 or a County Caseworker 1; orSuccessful completion of the County Social Casework Intern program; orSix months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; orAn equivalent combination of experien
