About the Position Meridian Home Group Home: Third-level Support Care position accountable for taking vital signs, assisting with care plan activities, observing and reporting patient conditions and behaviors, and participating in rehabilitation, ambulation, and transfers. May mentor or direct the work of lower-level employees. Communicates with doctors, patients, and families, and educates patients on care and dietary needs. May provide transport to off-site appointments and application of restraints when necessary. What you'll need to be Successful Great communication skills, management skills, organization skills, ability to multitask, and works well with a team. Preferred Qualifications Typically requires High School Diploma or high school equivalency, and 2-3 years of experience. May require a Certified Nurse Assistant (CNA) designation.
