About the Position

WORK LOCATION: COMMUNITY SUPPORT PROGRAM - Long Beach
PAY: $ 40,286.40 annually
SCHEDULE/SHIFTS: Monday through Friday, 8:00 a.m. to 4:30 p.m., and with variations as required to meet program needs.
RESPONSIBILITIES: Serves as IDD Targeted Case Manager (TCM) for people enrolled in the IDD Community Support Program (1915i). Manages and maintains caseload of people receiving IDD Community Support Program services. Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload. Revises/updates each person's PSS at least annually or when changes in the person's circumstances occur or when requests are made by the person/legal representative. Coordinates and monitors identified supports and services through regular monthly contact, face to face quarterly reviews, and annual assessment and re-certification meetings. Ensures services are adequate, appropriate, and meet the person's needs. Attention to detail, organizational and time-management skills, as well as the ability to articulate with written and verbal communication are required. Applies pertinent state and federal regulations as well as agency and program policy and procedures. Travel within the six southern counties is required. Must maintain eligibility to drive a state vehicle.
Requires a Bachelor's Degree in psychology or related mental health field and 4-7 years of experience. Must be eligible to drive a State of Mississippi vehicle.
