Coordination and Support of Complex Cases
·       Coordinate complex DD SOP Crisis cases, ensuring smooth acceptance, discharge, service coordination, and communication with all involved parties.
·       Serve as liaison across departments, providers, families, and agencies while delivering accurate information and participating in all required treatment planning.
Support a small, high intensity need caseload
·       Serve a small, high intensity need caseload that will include those in DD State Operated Programs Crisis Beds transitioning to the community as well as Behavioral Health State Facility discharges. The assigned caseload from DDD will be from Higginsville Habilitation Center, Optimistic Beginnings, and Fulton State Hospital.
·       The individual will consult with the assigned TCM Team for discharges from DMH/Division of Behavioral Health facilities.
·       Collaborate with an interdisciplinary team on a routine basis
·       Participate in the discharge process of individuals
Serve as a specialist in statewide support coordination practices
·       Serve as a statewide specialist in support coordination practices, providing coaching, training, and clear communication on policies and procedures.
·       Build strong relationships with stakeholders, identify inconsistencies, and collaborate with teams to create unified solutions.
·       Attend required meetings and professional development while presenting information to stakeholder groups as needed.

Minimum Qualifications:
Bachelor's Degree or Registered Nurse license with 4-5 years of relevant experience.
