About the Position
This Time-Limited (14) position will assist families of infants and toddlers with disabilities in accessing health and developmental services and make referrals to providers for needed services; · Provide targeted case management by coordinating, facilitating, and monitoring the provision of early intervention and other services under Part C of IDEA, including evaluations and assessments; · Facilitate and participate in teams to develop, review, and evaluate Individual Family Service Plans (IFSP) and ensure appropriate services are being provided; · Inform families of their rights and procedures for safeguarding their rights and maintain early intervention records for accuracy and completeness; · Assist families in achieving their goals, including understanding their child's strengths, abilities, and special needs and how to help their child develop and learn; · Facilitate the development of a transition plan to preschool, school, or other appropriate services; and · Conduct outreach into the community to ensure all infants and toddlers with disabilities are identified and participate with community stakeholders to enhance the early intervention system.
