THE POSITION
This case manager position serves as the primary case manager for individuals who have a mental illness or intellectual disability and who are involved in the adult criminal justice system. Come join our dedicated team and help make a difference in the lives of others.
DESCRIPTION OF WORK
The case manager is responsible to advocate and assist consumers in gaining access to needed medical, social, educational, and other services. The outcome of these services will focus on integration in the community, vocational/educational participation, and adequate social supports to successfully complete the conditions of their probation. The case manager functions in a team modality to determine the least intrusive and least restrictive method that can be employed in providing the treatment necessary to manage symptoms of mental illness and behavior that result in recovery and wellness and provide protection for the individual served, their family, and the community. This is a highly responsible supportive counseling/case management position in the Forensic Unit.Responsibilities include developing individualized service plans; reviewing the consumers' receipt of and participation in services; reviewing clinical assessment information; assisting consumers in establishing informal support networks; assisting consumers in identifying, accessing, and learning to use community resources; submitting necessary documentation; and other duties as assigned.Work Schedule and Additional Information:Full time employment, 8:30 am through 4:30 pm, Monday-Friday, with a 30 minute lunch.Telework: You will not have the option to telework in this position.You will receive further communication regarding this position via email. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONSMinimum Experience and Training Requirements:Six months of experience as a County Caseworker 1; orSuccessful completion of the County Social Casework Intern program; orA bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; orAny equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related soci
