DESCRIPTION OF WORK Responsible for case management services, including maintaining close contact with consumer families and service providers in order to evaluate and document treatment being provided and progress in meeting identified goals of treatment. May serve as a lead worker for professional staff and student interns. Advocates for referral for collateral/supportive services in order to meet clients' needs in areas other than behavioral health, including but not limited to economic, health, physical, or vocational needs. Provides frequent direct contact and treatment at least biweekly, but as needed by the consumer. Provides crisis intervention and emergency services as needed to the Blended consumers. Provides 24 hour / 7 days per week on-call coverage on a rotating basis. Obtains information from consumers, members of their families, and others necessary for the identification of social, economic, emotional, health, or physical problems and for assisting consumers in obtaining a wide variety of services on the basis of established policy, regulations, and statutes.Work Schedule and Additional Information:Full-time employment.Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch. Irregular hours, including evenings and weekends, may be required.After completion of training, provides 24/7 on-call coverage on a rotating basis.May have occasional overnight travel or out of county travel for meetings or training.Telework: You will not have the option to telework in this position.You will receive further communication regarding this position via email. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONSMinimum Experience and Training Requirements:Six months of experience as a County Caseworker 1; orSuccessful completion of the County Social Casework Intern program; orA bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psyc
