DESCRIPTION OF WORK
This professional position is responsible for providing case management to individuals meeting criteria for developmental delays, mental health, and/or with intellectual disabilities.Responsible for monitoring, linking, and evaluating services.  Link to other services and make referrals as needed.  Provide regular direct individual contact and provide face-to-face case management services.  Meet productivity standards as set for the program.  Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family.  Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy.  Obtain and update information within required timeframes.  Provide crisis intervention services as needed.  Work in cooperation with and consult with Agency staff, supervisors, other providers, and community resources as needed.  Link individuals to transportation services or transport individuals to appointments as needed.  Attend meetings, staffings, and trainings as needed.  At times, may provide coverage in Somerset County.
