The Luzerne - Wyoming County, Area Agency on Aging (AAA) is seeking a hard-working Aging Care Manager 3 to assist senior citizens in obtaining services that help them stay active and safe. DESCRIPTION OF WORK As an Aging Care Manager 3, you will act as an Aging Protective Services Investigator in the Protective Services Unit. You will be responsible for investigating allegations of abuse, conducting personal visits with older adults, interviewing all collateral contacts pertinent to a case, and collecting evidence and documents. Work involves collaborating with other agencies and entities to determine case outcomes in terms of a substantiated or unsubstantiated case decision. A key duty of this role will be ensuring the safety of older adults, which will require you to assist with activities such as seeking medical intervention, applying for entitlement programs, as well as addressing eviction notices and placement needs. Additional responsibilities include:Using legal intervention, as needed, to gain access to the older adult or to participate in proceedings for appointment of a guardian of estate or personReferring cases to other agenciesProviding coverage on a rotating basis to receive new referralsConducting reassessmentsDocumenting all case activitiesVolunteering for AAA paid after-hour crisis coverageCompleting statistics, reports, case records, and additional forms Educating the general public and other agencies on AAA programs and Protective Services regulationsWork Schedule and Additional Information:Full-time employmentWork hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.Telework: You will not have the option to telework in this position.Salary: In some cases, the starting salary may be non-negotiable.You will receive further communication regarding this position via email.
