Luzerne County Area Agency on Aging is seeking a compassionate and knowledgeable Aging Care Manager 2 to help older individuals and their families navigate care options, access vital services, and maintain independence with dignity. This position gives you the opportunity to positively impact the lives of older adults while building a rewarding career in aging services within a supportive, mission-driven environment. DESCRIPTION OF WORK As an Aging Care Manager 2, you will be responsible for assessing the needs of consumers utilizing state mandated assessment forms. Work involves developing effective care plans to meet the needs of consumers and implementing the care plans in a professional manner. You will be expected to maintain a thorough knowledge of all AAA service programs and community support services in order to assist consumers in navigating eligible benefits. You will have the opportunity to assist consumers in negotiating the Aging service system by contacting providers concerning service delivery issues or a need for change in service delivery. Additional responsibilities include:Conducting field and home visits to determine a level of care and initiate care plansAssisting walk-in consumers with completion of forms and applicationsHandling follow up phone calls with the County Assistance office, hospitals, long-term care facilities, and other referral sourcesEntering data into the state-wide SAMS systemConducting reassessment activitiesArranging for financial management for consumers who are unable to handle their own financesAssisting with pre-placement and placement activities when community services are longer sufficient to maintain consumers in the communityWork Schedule and Additional Information:Full-time employmentWork hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.Telework: You will not have the option to telework in this position.Salary: In some cases, the starting salary may be non-negotiable.You will receive further communication regarding this position via email.
