We are looking for an Aging Care Manager that is interested in helping senior citizens discover resources that will enhance their quality of life. DESCRIPTION OF WORK
The primary responsibility of this role is to work with older adults, their families, and professionals, while providing ongoing care management support. This includes carrying out specialized tasks such as protective services, care transitions, and person-centered counseling. In this position, you will be responsible for managing a caseload of consumers and providing assistance in various areas as required. Additionally, you may need to coordinate services with physicians, community agencies, financial institutions, and other relevant stakeholders on a regular basis. Our ideal candidate must have the ability to handle multiple tasks and pay close attention to detail, specifically when completing paperwork, work assignments, and meeting deadlines. Work Schedule and Additional Information:Full-time employmentWork hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch. On-call hours may be required. Overnight and out of country travel for meetings and training may also be required.Telework: You will not have the option to telework in this position.Salary: In some cases, the starting salary may be non-negotiable. You will receive further communication regarding this position via email. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONSMinimum Experience and Training Requirements:Three years as an Aging Case Aide 2; orA bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; orAn equivalent combination of experience and training.Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.Other
