Within this role, you will act as a resource and advocate for both our clients and their families. DESCRIPTION OF WORK In this trainee position (probationary period of six months), this employee is specifically trained to assess the needs of consumers utilizing state mandated assessment forms, developing effective care plans to meet these needs, and implementing these care plans in a professional manner. An employee performance evaluation will be completed in six months. Upon successful completion of the six-month probationary period, the employee will be promoted to an Aging Care Manager 2.
